Before Your Arrival

We use the My CS-Link patient portal as our primary way to stay in touch with you. It’s the fastest, most secure way for us to share test results, send messages, and manage your care plan.

  • Set Up Your Account: Please create your My CS-Link account.
  • New Patient/Symptom Questionnaire: As your appointment date approaches, you will receive a link via text or email to complete your New Patient/Symptoms Questionnaire.

A Quick Tip: Completing these at home allows our team to review your history in detail before your appointment. If you aren’t able to finish them beforehand, don't worry! We will have a tablet ready for you, though it may add a bit of time to your check-in process.

When you come for your appointment, please bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required by your insurance)
  • Physical copies (CD-ROMs) or digital access to X-rays, MRIs, and CT scans, along with any relevant reports
  • List of medications (if any)

Upon Arrival

Please check in at the front desk. Even if you’ve already completed your online survey, we may have a few brief forms for you to sign to finalize your registration.

We recognize that your time is valuable and make every effort to stay on schedule. We sincerely apologize if we keep you waiting and appreciate your patience as we give every patient the dedicated, comprehensive time they need.

Rescheduling or Cancellations

If you need to move your appointment, please message us via My CS-Link or call the office during business hours. We kindly ask for at least 2 business days’ notice so that we can offer that time to another patient on our waiting list.
